Jonathan T. Hoch, Sr.

Executive Director, Italian Center for Stamford, Member of Planetree Patient and Family Partnership Council

Jon is the Executive Director Italian Center of Stamford, arole he has held since 2020. He has been an active member of the PlanetreeGlobal PFPC since 2020. Jon has served on many Council sub-committees andinitiatives, including work on DEI, Planetree’s new certification standards andas a speaker at the Planetree International Forum. His professional andnon-profit leadership experience has been an asset to Planetree as a member ofthe Planetree International Board of Directors, where he balances his legal andfiduciary role with keeping the voice of patients and families front andcenter.

 Jon’s journey as a patient advocate began as a teenager afflictedwith a rare blood disorder. He spent much of his teenage years as the oldestpatient in children’s hospitals throughout the Northeast United States. Aftercollege, Jon worked in a group home for abused and neglected boys in 1998. Hecontinued to work with underrepresented and disadvantaged youth in theeducation and Juvenile Justice programs for the next 20 years throughout CT.Jon’s formal journey as a Patient and Family Advocate began in 2011 after alengthy and arduous battle with lymphoma resulting in a four-month inpatienttreatment that allowed him to access every possible service a health systemoffers. Following this experience, Jon joined the Stamford Hospital Patient andFamily Partnership Council where, along with other Stamford Health Committees,he remains actively engaged. He is a recognized leader in the Stamfordcommunity, and a proud husband and father of two sons.
